It is Allan Myers’ policy that this training be conducted annually. False certification is punishable under Section 110 (a) and (f) of the Federal Mine Safety and Health Act.
This consent form expires one year from the date of submission. When you submit this form, it will not automatically collect your details like name and email address. Allan Myers team members can receive a site-specific helmet decal for the current year’s training once completed by visiting any of the scale houses.
